Fort Saskatchewan Care Community is a Designated Supportive Living community, offering 24-hour care from personal support workers and licensed nurses.
Residents are cared for by a healthcare team that includes Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Health Care Aids (HCAs).
Supportive living residents also have access to specialized services including Occupational Therapy, Practical Therapy, Social Services, and Dietician Services.
Fort Saskatchewan Care Community follows the Eden Model of Care. This philosophy supports our residents to continue to be active, healthy, contributing members of their community.
We are managed with a hospitality perspective focused on wellness rather than illness. We provide individuals with choices and believe that medical treatment is the servant of genuine human caring, rather than its master.
The Eden Model of Care believes that when an individual’s ability to contribute is recognized, the years to come can be a time of meaningful activities and satisfying personal growth.
Adults living in Designated Supportive Living (DSL) need significant support for many aspects of daily living, such as preparing meals, bathing, and grooming.
The DSL program is offered in partnership with Alberta Health Services. Eligibility is determined through an assessment process by AHS.
Designated Supportive Living services include:
Are you ready to start the process?
The first step is to call Alberta Health Services at 403-943-1920 and answer a series of questions on the phone to register for Home Care.
After that, you will be assigned an AHS Continuing Care Case Manager in your community, who will guide you through the process, assist with the Designated Supportive Living program, and help to choose a seniors community to live in.
We prepare high-quality cuisine in our on-site kitchen and serve our residents in their cottage dining area. Residents of each cottage dine together for meals creating a warm, family atmosphere.
Residents have access to snacks and beverages throughout the day in their cottage kitchen and are always welcome to help themselves.
Fort Saskatchewan Care Community does not work with large corporate food delivery systems. Instead, we create unique menus emphasizing local flavours and incorporating locally grown seasonal produce.
Special preparations (mincing, pureeing, etc.) and special dietary requirements (for diabetes, celiac, allergies, etc.) will be accommodated.
